About the RoleThis is an exciting opportunity to work as part of an interdisciplinary team at Mass General Brigham Home Care. As a Patient Care Specialist, you will collaborate with patients, family members, physicians, and other healthcare professionals to identify patient needs and coordinate services to meet those needs.
- Main Responsibilities:
- Provide physical therapy services to patients in their place of residence, under the direction of a registered physical therapist
- Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services
- Document patient information thoroughly and timely
- Two years of experience in acute care, rehab, or homecare as a Physical Therapy Assistant
- Licensed as a Physical Therapist Assistant in Massachusetts
- Excellent customer service skills and ability to work with diverse populations
