Job Title: Registered Nurse - Home Health

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role:

The Registered Nurse will be responsible for providing high-quality, patient-centered care to individuals in their homes. This will involve assessing patients' needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care coordination.

Responsibilities:
  • Provide comprehensive nursing care to patients in their homes, including medication management, wound care, and vital sign monitoring.
  • Assess patients' needs and develop individualized care plans to meet their unique needs and goals.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination.
  • Communicate effectively with patients, families, and caregivers to ensure that their needs are met and that they are informed and engaged in their care.
  • Document patient information accurately and thoroughly, including progress notes, medication lists, and care plans.
Qualifications:
  • Current RN licensure in the State of New Jersey.
  • At least 1 year of experience in home health or hospice care.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
