We are seeking a highly skilled Clinical Care Coordinator to join our team at FDIHB. As a Clinical Care Coordinator, you will play a critical role in coordinating patient care and ensuring that our patients receive the highest level of care.
Key Responsibilities- Develop and implement comprehensive care plans that meet the unique needs of each patient.
- Coordinate with healthcare providers, family members, and community resources to ensure seamless care transitions.
- Provide education and support to patients and their families to promote healthy behaviors and self-management of chronic conditions.
- Collaborate with interdisciplinary teams to identify and address patient needs, and to develop and implement strategies to improve patient outcomes.
- Monitor and evaluate patient progress, and make adjustments to care plans as needed.
- Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services and support.
- Participate in quality improvement initiatives to identify and address gaps in care, and to develop and implement strategies to improve patient outcomes.
- Two (2) years of experience as a Registered Nurse in a clinical, medical/surgical, acute, and/or critical care setting.
- Associate of Science degree in Nursing from an accredited nursing program.
- Valid and unrestricted Registered Nurse license in any U.S. State or Territory.
- American Heart Association (AHA) certification in Basic Life Support (BLS).
- Experience working with patients with chronic conditions.
- Knowledge of the Navajo Nation and its healthcare system.
- Fluency in the Navajo language.
