Role Expectations

As a Registered Nurse in Home and Community-Based Services, you will deliver and oversee nursing and personal care services to patients in their residences, following physician directives and adhering to relevant regulations and agency policies. You will collaborate with other healthcare professionals to ensure a seamless continuum of care.

Key Responsibilities

  • Deliver high-quality clinical services in accordance with the established care plan and infection control standards.
  • Conduct comprehensive nursing assessments in line with federal and state requirements.
  • Verify patient eligibility and medical necessity for services as per payer guidelines.
  • Develop and update individualized care plans in collaboration with community providers.
  • Adjust care plans as necessary to meet patient needs, with physician approval.

Qualifications

Experience Requirements

  • A minimum of one year of experience as a Registered Nurse is preferred.

Licensure Requirements

  • Current RN licensure in the state of practice is mandatory.
  • A valid driver's license, vehicle insurance, and access to reliable transportation are required.
  • Current CPR certification is essential.
