POSITION OVERVIEW:

ROLE SUMMARY

The Integrated Care Team is responsible for managing the treatment of both behavioral health (mental health and substance use) and physical health conditions. This is achieved through comprehensive care coordination, transitional care management, health promotion initiatives aimed at enhancing overall wellness, and monitoring critical health metrics through data analysis. The Care Coordinator plays a vital role in delivering health promotion activities, utilizing evidence-based practices in both individual and group settings. Additionally, the Care Coordinator will conduct screenings and assessments for clients interested in or currently enrolled in Integrated Care, ensuring that each individual receives the compassionate and competent care necessary for effective intervention and referral.

KEY RESPONSIBILITIES

  • Support participants and their families in developing self-management skills to enhance their health through education on disease management techniques.
  • Provide wellness coaching, empowering families and participants to advocate for themselves and access appropriate medical care.
  • Facilitate wellness and prevention groups utilizing evidence-based self-management models, adhering closely to the established frameworks.
  • Educate participants on navigating the healthcare system, offering guidance and support to them and their caregivers.
  • Utilize personal health information to educate consumers, assisting them in leveraging this information for self-management.
  • Ensure coordinated care for participants by collaborating with service providers and developing strategies to eliminate barriers to optimal health.
  • Facilitate access to necessary services during transitions between different levels of care.
  • Adhere to annual Key Performance Indicator evaluations.
  • Maintain regular and reliable attendance.
  • Accurately document all information in the Electronic Health Record (EHR).
  • Promote a culture of collaboration and cooperation with other departments, community organizations, and the public.
  • Assist participants in applying for Social Security Income (SSI) or Social Security Disability Insurance (SSDI) using the SOAR model.
KNOWLEDGE, SKILLS & ABILITIES
  • Strong communication skills, both written and verbal, across various platforms.
  • Ability to work independently as well as collaboratively within a team.
  • Critical thinking skills and the ability to utilize data to support participants.
  • Experience in providing wellness coaching and health promotion education.
  • Commitment to the principles of mental health recovery.
QUALIFICATIONS

Education

Bachelor's degree in social work, counseling, psychology, or a related health field is required.

Experience
  • Experience in a physical health care or behavioral health care setting is essential.
  • Experience in both physical and behavioral health care environments, particularly with adults facing mental health challenges, is highly desirable.
