Job Summary

We are seeking a highly skilled and dedicated Staff Pharmacist to join our team at Trinity Health. As a key member of our pharmacy team, you will play a vital role in providing exceptional patient care and contributing to the success of our organization.

Key Responsibilities
  • Develop and implement pharmacotherapeutic regimens and provide counseling to patients, utilizing pharmacology, pathophysiology, pharmacokinetics, and interpersonal communication skills.
  • Provide clinical pharmacy and drug information services to hospital staff, collaborating with physicians to ensure effective pharmacotherapy.
  • Compound, package, and dispense medications and IV solutions and preparations as ordered, ensuring patient safety and accuracy.
  • Supervise pharmacy technicians and interns, providing guidance and support to ensure proper training and job performance.
  • Act as a resource to students of various health disciplines, promoting education and professional growth.
  • Participate in quality improvement initiatives and professional growth to meet hospital, departmental, and personal goals.
  • Work collaboratively with physicians, nurses, and other staff to ensure effective pharmacotherapy and patient care.
  • Ensure patients are knowledgeable about new medications and the side effects of all medications they are taking.
  • Perform provider and patient education in a manner that is service-oriented and consistent with Trinity Health values.
  • Participate in community outreach as a member of Trinity Health and the Department of Pharmacy, promoting community service and engagement.
  • Carry out specific clinical protocols, such as IV-to-PO conversions and therapeutic substitutions, as defined by Pharmacy and Therapeutics.
  • Maintain required records and automated dispensing cabinet inventories for controlled substances, investigating discrepancies as needed.
  • Design pharmacokinetic dosing regimens and pharmaceutical care plans, documenting plans and interventions as required.
  • Obtain and maintain competency in accessing and documenting patient information in the automated patient medical record, as defined by departmental policies.
  • Explore ways to expand clinical pharmacy services and relationships, fostering growth within the department, Trinity Health, and the community.
  • Demonstrate self-directed learning and participation in continuing education/research to ensure professional growth.
  • Actively promote and participate in various task forces, teams, or committees in the department or for the hospital at large.
  • Participate in various events representing Trinity Health to the community, promoting the organization's mission and values.
Requirements
  • BS degree in Pharmacy, licensed to practice in the State of Michigan.
  • Advanced degrees (i.e., Doctor of Pharmacy) and residency training preferred.
  • Demonstrated ability to interact appropriately and effectively with patients, family members, healthcare team members, ancillary departmental staff, medical staff, visitors, and the public at large.
  • Demonstrated ability to perform duties in a safe manner, adhering to policies and procedures in the delivery of patient care.
  • Able to identify patient care concerns and problems outside the scope of responsibility and initiate appropriate corrective action.
  • Ability to change work priorities in consultation with supervisor as the situation warrants, adapting to changing patient and department needs.
  • Articulate and demonstrate commitment to the philosophy, values, mission, and vision of Trinity Health.
  • Demonstrated commitment to hospital pharmacy practice, as demonstrated by recent (within 5 years) employment in hospital practice or advanced clinical pharmacy work.
