Youth Care Specialist

We are seeking a skilled Youth Care Specialist to join our team at the Grand Lake Mental Health Center. As a vital member of our Integrated Team, you will provide targeted support services to youth and families, with a focus on preventing out-of-home or out-of-school placements.
About the Role
The successful candidate will have a strong background in psychology, social work, or a related field, with a minimum of one year's experience in the mental health sector. You will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including clients, families, and healthcare professionals.
Key Responsibilities
- Conduct comprehensive assessments of client needs and develop personalized care plans to address their requirements.
- Coordinate and facilitate wraparound/family teams to ensure seamless service delivery and support.
- Maintain close communication with the Integrated Team Manager and other clinical staff to guarantee high-quality services.
- Provide crisis intervention services on a 24-hour basis, as required.
- Monitor and evaluate services provided to clients, making adjustments as necessary to optimize outcomes.
Qualifications and Requirements
- Bachelor's degree in Psychology, Social Work, Sociology, or a related field.
- Minimum one-year work experience in the mental health sector, preferably in case management or a related role.
- Certification as a Behavioral Health Case Manager II.
Salary Information
The estimated salary for this position is $45,000 - $55,000 per annum, depending on experience and qualifications. Benefits include comprehensive health insurance, paid time off, and opportunities for professional development.
