Position Overview

Woven Care is in search of a dedicated Outpatient Speech Language Pathologist to provide exceptional care for our pediatric patients.

Key Responsibilities
  • Collaboration: Work closely with families, healthcare providers, educational institutions, and our multidisciplinary team to ensure optimal clinical outcomes and continuity of care.
  • Case Management: Engage in comprehensive case management activities that align with departmental quality assurance and improvement standards.
  • Patient Care: Conduct thorough diagnostic evaluations adhering to contemporary, evidence-based practices in speech-language pathology while incorporating cultural competence and family-centered care principles.
  • Evidence-Based Practice: Deliver services that reflect current best practices in speech-language pathology, ensuring compliance with departmental quality assurance standards.
Qualifications
  • Education: Master’s degree in Speech-Language Pathology.
  • Certifications: BLS for Healthcare Providers/CPR; ASHA Certificate of Clinical Competence (CCC) in Speech-Language Pathology.
  • Licensure: Valid Speech-Language Pathologist license from the State of Colorado; credentialing must be completed within three months of employment.
Physical Requirements

This role necessitates the ability to sit for 1-2 hours and stand or walk for 4-6 hours daily. Frequent bending and occasional lifting, carrying, pulling, pushing, and reaching are required. Precise motor skills with both hands are essential, and the ability to lift up to 50 pounds may be necessary.
