We are seeking a skilled Physician to join our team at CenterWell Senior Primary Care. As a Physician, you will play a vital role in providing high-quality, patient-centered care to our senior patients.

• Provide comprehensive medical care to patients, including wellness visits, physical exams, and chronic condition management.
• Develop and implement individualized care plans to meet the unique needs of each patient.
• Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
• Stay up-to-date with the latest medical research and best practices to provide evidence-based care.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
• 5 or more years of technical experience in a primary care setting.
• Licensure requirements of the state of jurisdiction.
• Graduate of an accredited MD or DO program.
