As a vital member of the healthcare team, the Care Manager III plays a crucial role in the comprehensive management of patient care.

- Develop and assess the Plan of Care tailored to meet individual patient needs.
- Facilitate connections to community resources aimed at enhancing patient satisfaction and health outcomes while minimizing healthcare expenses.
- Delegate responsibilities to appropriate team members to address patient needs efficiently.
- Ensure compliance with professional standards and regulatory requirements in all services provided.
- Implement discharge plans in agreement with patients, families, and physicians.
- Provide educational support to patients, families, and healthcare team members.
- Act as a liaison among patients, consulting physicians, and external agencies to coordinate appointments and necessary procedures.
- Maintain accurate patient accounts by collecting and updating personal and financial information.
- Offer empathetic assistance to patients and their families, addressing concerns with care.
Qualifications:
- Associate's Degree - Required
- Bachelor's Degree - Preferred
- Registered Nurse (RN) - Required
- Certified Case Manager (CCM) - Preferred
- Basic Life Support (BLS) certification from the American Heart Association - Required
- Minimum of 5 years in Nursing/Patient Care - Required
- At least 1 year in Case Management - Preferred
Travel Requirements:
Up to 20% travel may be necessary.
