About the Role

We are seeking a skilled Physical Therapist to join our team at Assured Home Health, a part of LHC Group. As a Physical Therapist, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Evaluate patients' functional status and physical therapy needs, working closely with physicians to develop personalized therapy plans.
  • Provide services within the scope of practice, in accordance with state laws and in coordination with the healthcare team.
  • Observe and report patients' responses to treatment, ensuring timely communication with supervising nurses and physicians.
  • Treat patients to relieve pain, develop and restore function, and maintain maximum functioning, adhering to acceptable standards of practice.
  • Instruct and inform healthcare team members, patients, and caregivers regarding therapy plans, functional limitations, and progress toward goals.
  • Visit patients according to Plan of Treatment, complete progress notes, and submit reports as per LHC policy.
  • Complete patient evaluations and develop PT plans of care within state-specific guidelines, reporting outcomes, goals, and anticipated frequency of care.
Requirements
  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification.
  • A valid driver's license and vehicle insurance, with access to a dependable vehicle.
