**Job Summary:**

We are seeking a skilled and compassionate Care Coordinator- LVN/RN to join our team at Sante Health System in Fresno, CA.

The estimated salary for this position is around $65,000 - $90,000 per year, depending on experience.

This role plays a critical part in our Chronic Care Management program, working closely with site staff, physicians, and management to ensure quality, cost-effective care for patients with complex medical conditions.

You will be responsible for coordinating medical management, promoting patient engagement and satisfaction, increasing physician and staff satisfaction, and maximizing the use of available resources.

The ideal candidate will have a graduation from an accredited school of nursing, a minimum of five years of professional experience in an ambulatory clinic or hospital setting, and an active State Registered LVN license.

Responsibilities:

  • Conduct comprehensive initial assessments and develop proactive care plans in collaboration with patients, physicians, and other care team members.
  • Ensure accurate coding for maximum financial capture and maintain patient documentation in the medical record.
  • Work directly with involved parties to verify benefits and eligibility for medical necessity and identify eligible patients for CCM and PCMH programs.
  • Conduct ongoing assessments to determine response to care plan or services and reprioritize care plans as needed.
  • Generate statistical reports and maintain patient demographics, problems, diagnoses, medications, lab results, and short-term/long-term goals.
  • Assist and provide education to staff and physicians in resolving questions or problems involving patient treatment plans.
  • Maintain confidentiality and comply with HIPAA regulations.
  • Participate in committees, attend required meetings, and engage in professional development to maintain affiliation.
