Vaya Health is seeking a skilled Transition Coordinator LP to join our team. As a key member of our transition planning process, you will work closely with members, their families, and healthcare providers to ensure a smooth transition from institutional settings to community-based care.
Key Responsibilities:
- Develop and implement individualized transition plans that address the unique needs of each member
- Collaborate with healthcare providers, social services, and other stakeholders to ensure a comprehensive transition process
- Provide education and support to members and their families to promote self-advocacy and independence
- Document all transition activities and outcomes in a clear and concise manner
Requirements:
- Master's degree in a human services field with clinical licensure (LCSW, LCMHC, LPA, or LMFT)
- 2+ years of experience working with the population served
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
