  • Document all transition activities and outcomes in a clear and concise manner

Requirements:

  • Master's degree in a human services field with clinical licensure (LCSW, LCMHC, LPA, or LMFT)
  • 2+ years of experience working with the population served
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
