Healthcare Team Assistant

We are seeking a skilled Healthcare Team Assistant to join our team at Sea Mar Community Health Centers. In this role, you will support the care team to ensure that patients receive timely and courteous primary care services.
**Key Responsibilities:**
- Assist with provider referrals and scheduling appointments
- Receive, document, and return patient messages
- Coordinate needed services both internally and externally
- Manage quality gap reports and contact patients for follow-up appointments
- Perform QI audits and participate in organizational committees as assigned
**Requirements:**
- High school graduation or equivalent required
- Previous experience in Family Medicine preferred
- Bilingual skills in English/Spanish preferred but not required
