Nurse Practitioner

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team in providing high-quality home risk assessments and care management services. As a Nurse Practitioner with Matrix, you will have the opportunity to work with a diverse range of patients, conducting comprehensive health assessments and developing personalized care plans to improve health outcomes.
Key Responsibilities:
Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
Provide health education and collaborate with primary care physicians on patient care
Work collaboratively with case managers, social workers, and other healthcare professionals to ensure comprehensive care
Conduct assessments in a variety of settings, including home visits, skilled facilities, and telehealth visits
Requirements:
Master's degree in nursing or commensurate experience and satisfactory completion of NP licensure
Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
1 year of experience as a Nurse Practitioner
Strong computer skills and familiarity with electronic health records software
Excellent verbal and written communication skills
