Hospice Care Specialist

At (404100) Medi Home Hospice, we're dedicated to delivering exceptional care to our patients and their families. As a Full-Time Registered Nurse, you'll play a vital role in our multidisciplinary team, working under the guidance of our Director of Nursing or Hospice Administrator.

The ideal candidate will possess a strong background in direct patient care, with a focus on hospice nursing experience. You'll be responsible for:

  • Developing and implementing personalized care plans to optimize patient comfort and health.
  • Collaborating with our multidisciplinary team to ensure seamless care delivery.
  • Communicating effectively with patients, families, and healthcare professionals to address their needs and concerns.
  • Providing supportive care in accordance with the attending physician's orders.
  • Participating in on-call rotations as needed.

With a minimum of one-year experience in direct patient care, you'll bring a unique perspective to our team. We encourage applicants with a strong work ethic, excellent communication skills, and a passion for delivering compassionate care.

We offer a competitive salary range of $85,000 - $115,000 per year, commensurate with experience. Our benefits package includes:
