Position Overview:We are seeking a Licensed Practical Nurse to become a vital part of our team.
In this role, you will report to the Executive Director or Administrator while working under the guidance of a Registered Nurse to deliver direct skilled nursing care at the bedside. You will play an active role in coordinating all facets of patient hospice care.
- Provide direct care services to hospice patients and offer supportive assistance to their families with a manageable caseload.
- Assess patient and family needs, contributing to the development and implementation of individualized care plans.
- Communicate any findings or changes in patient conditions to the Registered Nurse, interdisciplinary team, and referring physician, ensuring all observations are documented accurately.
- Demonstrates compassion, kindness, and empathy towards patients and their families.
- Graduated from an accredited Practical Nursing program and holds a current nursing license in the state of employment.
- Possesses 1+ years of nursing experience in hospice, home health, or oncology settings preferred.
