Job Summary

Vinfen is seeking a Licensed Practical Nurse (LPN) II to join our team in a medically intensive program designed to meet the needs of individuals with brain injuries, developmental delays, or intellectual disabilities. As a LPN II, you will provide direct service and daily nursing care, ensuring that care is provided in a respectful, dignified, and high-quality manner.

Responsibilities
  • Implement person-centered treatment care plans thoroughly in accordance with medical orders.
  • Provide assistance with activities of daily living, including showering, dressing, shaving, mouth care, foot care, and diabetes care.
  • Clinically supervise Certified Nursing Assistants (CNAs) during designated shifts and provide training as needed.
  • Document nursing care and treatments as ordered by primary care providers according to Vinfen policy and procedure.
  • Ensure the completion of all skilled nursing care needs of individuals served, including wound care, GT/JT care, trach care, and nutritional support.
  • Monitor and observe clients for any change in status and report issues in a timely manner to the Program Director, Supervising Program Director (if needed), and Program Nurse.
  • Ensure that the environment is safe, clean, comfortable, and welcoming to individuals and families and/or guardians and other significant individuals and visitors.
  • Advocate effectively for people served.
Requirements
  • LPN credentials in Massachusetts required.
  • A high school diploma or equivalent is required; an Associate's degree preferred.
  • One to two years of supervised practical experience, including internships in a clinical or rehabilitation setting is helpful, but not required.
  • Experience working with people with developmental and/or psychiatric disabilities, medical illnesses, and other co-occurring disorders preferred.
  • Must be able to become CPR certified within 2 weeks of hire.
  • May be asked to obtain a Certified Brain Injury Specialist (CBIS) certification.
  • A valid driver's license must be presented at the time of employment.
