- Documentation and Care Delivery
- Provide high-quality clinical services within the scope of practice, adhering to infection control standards and payer requirements.
- Accurately and timely complete comprehensive assessments, including medication reconciliation, and document patient visits per policy.
- Quality
- Ensure patients meet home health eligibility and medical necessity guidelines, accurately determining primary focus of care and developing plans in collaboration with physicians.
- Communicate relevant information effectively with agency staff, physicians, patients, and family members to ensure quality care and service excellence.
- Professionalism and Teamwork
- Exhibit a positive attitude, willingness to learn, and punctuality for all meetings and tasks.
- Participate in on-call rotation, weekend rotation, and survey readiness activities, adhering to the agency's utilization management model.
- Current RN licensure in the state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
