Position Summary: The Care Transition Nurse Specialist plays a crucial role in managing patient care during the critical period following hospital discharge. This position is designed to enhance patient outcomes through effective disease management and care coordination.
Compensation and Benefits:
- Hourly wage ranges from $25.74 to $42.03, determined by prior experience.

Key Responsibilities:
- Facilitate disease management and care coordination for patients post-discharge.
- Collaborate with various healthcare providers, including home health agencies and skilled nursing facilities, to enhance patient outcomes and minimize preventable readmissions.
- Provide insights on quality metrics and program effectiveness, while also developing educational initiatives for post-acute care providers.
- Deliver direct patient care and transitional support as needed.
- Work in tandem with clinical teams to address inpatient needs and ensure a seamless transition of care.
- Lead interdisciplinary meetings to foster collaboration among all care providers.
- Oversee a team of nursing staff and social workers in the absence of the Transition of Care Supervisor.
- Address complex medical and social issues that require effective triage and communication across various healthcare entities.
- Accept referrals for complex problem resolution, necessitating advanced problem-solving skills and coordination across the care spectrum.
Qualifications:
Education:
- Bachelor of Science in Nursing (BSN) is required; however, an Associate Degree in Nursing will be considered if the candidate commits to obtaining a BSN within 12 months of hire.
Experience:
- A minimum of three years of experience as a Registered Nurse is required, with a preference for candidates possessing 1-2 years of experience in case management or care transitions.
Licensure and Certification:
- Must hold a current, unrestricted RN license in the state of Oklahoma.
- Basic Life Support (BLS) certification is mandatory and must be maintained throughout employment.
