As a Registered Nurse in the Hospice Care team at Northeast Georgia Health System, you will play a vital role in providing compassionate and high-quality care to patients and their families. Your expertise will be essential in delivering patient-centered care, collaborating with the interdisciplinary team, and ensuring the best possible outcomes for our patients.
Key Responsibilities:- Deliver patient care in accordance with the standards of care defined within the American Nurses Association (ANA) standards of Clinical Practice and Standards of Hospice Nursing Practice and Professional Performance.
- Assess, instruct, and refer as indicated to ensure a safe environment for the patient.
- Collaborate with the attending physician, hospice Medical Director, referral source, and hospice intake department to ensure effective patient care delivery and the achievement of desired patient outcomes.
- Provide education and instruction to patients and/or caregivers appropriate to the learning ability/level of the patient and/or caregiver.
- Document care in accordance with medical documentation standards and in accordance with agency policy, standards, and timelines.
- Licensure or other certifications: Registered Nurse in Georgia; Current BLS certification
- Educational Requirements: Associates Degree
- Minimum Experience: One (1) year in a health care profession for RN; experience in hospice, oncology, or home health
- Preferred Licensure or other certifications: CHPN
- Preferred Educational Requirements:
- Preferred Experience: Emphasis on care of terminally ill patients, adults in acute care medical and surgical areas/home care preferred. Additional experience with children and infants desired. One year hospice experience preferred.
