Behavioral Health Specialist

We are seeking a skilled Behavioral Health Specialist to join our team at Quartet Health. As a key member of our multidisciplinary team, you will be responsible for providing holistic care to individuals with serious mental illness. Your expertise will be instrumental in managing whole-person care, addressing non-clinical care gaps, and facilitating connections to community-based services and supports.
Key Responsibilities:
- Conduct assessments to identify non-clinical care gaps and develop plans to address them
- Collaborate with the care team to submit orders and referrals to external partners and follow up on their status
- Advocate for patients by helping them complete applications and forms to address their needs
- Ensure patients engage with resources identified and provided
- Participate in patient rounding meetings to drive care planning
Requirements:
- Bachelor's or Master's degree in Social Work
- 5+ years of clinical social work experience
- Excellent written and verbal communication skills
- Ability to develop and implement relevant care plans

Preferred Qualifications:
- Master's degree in Social Work
- Experience in caring for patients with serious mental illness and co-occurring substance use disorders
- Bilingual candidate (Spanish and/or other languages)
