Rehabilitation Practitioner

The University of Vermont Health Network is seeking a skilled Rehabilitation Practitioner to join our Rehabilitation team. As a Rehabilitation Practitioner, you will be responsible for conducting occupational therapy assessments, developing and implementing treatment plans, and re-evaluating patient status to restore function and maximize ability following disease or disability.

Key Responsibilities:

• Conduct comprehensive occupational therapy assessments to identify patient needs and develop individualized treatment plans.
• Implement evidence-based interventions to promote patient independence, mobility, and participation in daily activities.
• Collaborate with interdisciplinary teams to ensure seamless care coordination and optimal patient outcomes.
• Provide education and training to patients and their families on proper techniques and strategies for managing chronic conditions.
• Maintain accurate and detailed records of patient progress, including assessment results, treatment plans, and outcome measures.

Requirements:

• Bachelor's or Master's degree in Occupational Therapy from an accredited institution.
• Valid and current New York State Occupational Therapy license and registration with the National Board Certification for Occupational Therapy.
• Ability to physically treat an average of 12 inpatients or 15 outpatients per day and complete accurate and legible documentation as per established guidelines.
• Ability to qualitatively evaluate and treat all age groups and diagnostic categories in both an inpatient and outpatient setting.
• Ability to communicate effectively with patients and caretakers regarding anticipated Occupational Therapy treatment plan, level of expected patient participation, and expected outcome.
• Ability to communicate effectively with the public and the Medical/CVPH staff as a member of the Rehabilitation team.

Salary Information:

$42.34 - $44.88 per hour (Hourly Salary)
