Job SummaryThe primary responsibility of this position is to provide Community Support Team services, including direct and indirect interventions, to individuals with complex and extensive treatment needs. You will work collaboratively with a team of professionals to develop and implement comprehensive service plans, provide psycho-education, and assist with crisis interventions.
Key Responsibilities- Develop and implement Person-Centered Plans (PCPs) in collaboration with the individual, their family, and other stakeholders.
- Provide psycho-education and support to individuals and their families to promote recovery, symptom reduction, and increased coping skills.
- Assist with crisis interventions and provide first responder support on a 24/7/365 basis.
- Coordinate services with medical and non-medical providers, engage community and natural supports, and include their input in the PCP.
- Monitor and document the status of the individual's progress and the effectiveness of the strategies and interventions outlined in the PCP.
- Coordinate service user transition to another type of service.
- Master's degree in a human service field or a related field.
- At least 1 year of documented experience working with the adult MH/SA population.
- Valid NC driver's license and personal vehicle insurance coverage.
