We are seeking a compassionate and dedicated Nurse Practitioner to join our team of healthcare professionals.
This role will involve providing high-quality patient care through evaluation, assessment, and management of health problems.
The successful candidate will work collaboratively with an interdisciplinary team to develop and implement individualized plans of care that address the physical, emotional, and spiritual needs of patients and their families.
Responsibilities:
Provide medical care and manage illness in compliance with state regulations under the supervision of a collaborating physician.
Consult with attending physicians, medical directors, and other healthcare practitioners regarding patient eligibility for care and ongoing care needs.
Review clinical information, coordinate patient care, and oversee case management in consultation with the interdisciplinary team.
Educate and counsel patients, families, and caregivers on preventative care, medical problems, and psychological/spiritual concerns.
Maintain knowledge of current Medicare/Medicaid, state/federal rules, and regulations for services.
Manage documentation for testing, labs, and medical records.
Participate in staff meetings and contribute to quality improvement initiatives.
