Healthcare Navigator

Estimated Salary: $65,000 - $90,000 per year.

We are seeking a highly skilled and compassionate Healthcare Navigator to join our team at Steward Health Care. As a key member of our care coordination team, you will play a vital role in ensuring the smooth transition of patients from hospital to home or other healthcare settings.

In this role, you will work closely with our interdisciplinary team to develop and implement individualized discharge plans that meet the unique needs of each patient. You will be responsible for coordinating with physicians, social workers, and other healthcare professionals to ensure seamless communication and collaboration.

Key Responsibilities:

• Conduct thorough assessments of patients' medical and social needs
• Develop and implement effective discharge plans that address patients' complex needs
• Collaborate with healthcare teams to ensure timely and safe transitions of care
• Provide education and support to patients and their families throughout the discharge process

Requirements:

• Bachelor's degree in nursing or related field
• Current licensure as a registered nurse in Massachusetts
• Experience in acute care setting and/or managed care environment
• Excellent communication and interpersonal skills
