Job Summary

We are seeking a skilled Community Health Worker to join our team at Alliance. This hybrid role will allow the successful candidate to work remotely on certain days while also requiring in-office presence as approved by their supervisor. The Community Health Worker will use engagement strategies and strong community connections to assist members with resolving their unmet health needs.

Key Responsibilities
  • Assess and address unmet health needs through engagement strategies and community connections
  • Provide education and support to help members understand their benefits
  • Collaborate with the Care Team, Community Health and Well-Being, and Alliance Departments to maximize member experience and build social capital
  • Develop in-depth knowledge of various community systems and provide consultation and technical assistance to MCO clinical departments
  • Engage providers and external stakeholders to develop connections and increase community inclusion outcomes
Requirements
  • High school diploma or GED and a minimum of two years of experience working with individuals/with behavioral health needs, or minimum of four years lived experience in navigating any of the Mental Health, Public Health, Social Service, and/or Justice systems
  • NC Community Health Worker Certification within 12 months of hire
  • Valid NC Driver license
Preferred Qualifications
  • Completion of training and/or documented knowledge of WRAP; Person-Centered Thinking; WHAM (Whole Health Action Management), Trauma Informed Care; MH First Aid; IPS-SE; Community Inclusion/Integration; Harm Reduction; Recovery Model
