We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Field-Based RN Case Manager. This is a unique opportunity to work in a dynamic and patient-centered environment, where you will have the flexibility to work in the field and telecommute.
Key Responsibilities- Assess, plan, and implement care strategies for Medicare and Medicaid members, ensuring they receive the most appropriate level of care.
- Proactively engage members to manage their own health and healthcare, utilizing Motivational Interviewing Skills.
- Manage transitions of care for members upon discharge from in-patient hospital stays and emergency rooms.
- Help members set goals and develop care plans to achieve those goals, updating plans regularly as progress is made.
- Support members in improving their well-being by staying out of the hospital and attending regular visits to their primary physician and specialists.
- Identify and initiate referrals for social service programs, including financial, community resources, medical, behavioral health, psychosocial, community, and state supportive services.
- Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
- Advocate for members as needed to ensure they have access to care.
- Provide clinical support for non-clinical staff, educating them on medications, diagnoses, and disease-specific education.
- Collaborate with the interdisciplinary team to ensure appropriate services and identify needs.
- Current, unrestricted RN license in the state of Kansas.
- 2+ years of experience working in the community as a RN in acute in-patient care, long-term care, home health, hospice, public health, behavioral health, or assisted living.
- Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook.
- Ability to travel up to 75% of the time with reliable transportation, a current and non-restricted state of Kansas Driver's License, and State-required insurance.
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI).
- Live in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service.
- Bachelor's degree in nursing.
- A background in managing Medicaid and Medicare populations with complex medical and behavioral needs.
- Experience with arranging community resources.
- Experience with electronic charting.
