**The Role:**
We are currently seeking an experienced Hospice Registered Nurse / RN Case Manager to join our team. This role involves working closely with patients, families, and healthcare teams to deliver exceptional care and service.
**Responsibilities:**
- Provide comprehensive patient assessments and develop individualized care plans.
- Collaborate with interdisciplinary teams to ensure seamless coordination of care.
- Communicate effectively with patients, families, and healthcare providers.
- Maintain accurate and timely documentation of patient records.
