About the Role

MaineHealth is seeking a highly skilled Physical Therapist to join our Home Care team. As a Home Care Physical Therapist, you will play a critical role in supporting our mission to deliver high-quality, patient-centered care to our community.

Key Responsibilities

  • Provide highly reliable, evidence-based physical therapy services to patients in their homes, with a focus on improving functional ability and quality of life.
  • Conduct thorough assessments, develop and implement individualized treatment plans, and evaluate patient progress.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Stay up-to-date with national standards of practice and Maine State Board of Licensure guidelines to ensure compliance and quality care.

Requirements

  • Graduation from an accredited physical therapy program.
  • State of Maine licensure as a Physical Therapist.
  • Current, valid driver's license and liability insurance.
  • Current certification in Basic Life Support for Healthcare providers.
  • Minimum one year of full-time equivalent experience in a supervised setting.
  • Excellent interpersonal and communication skills to interact effectively with patients, families, and healthcare team members.
  • Strong analytical ability to identify changes in patients' conditions and initiate appropriate action.
