  • Assists patients in accessing necessary entitlements, resources, and psychosocial referrals.
  • Participates in Quality Assurance and Utilization Review initiatives as directed.
  • Empowers patients to make informed decisions regarding their care plans.
  • Maintains precise and timely patient information, ensuring accessibility for review and compliance with reporting requirements.
  • Facilitates collaborative relationships within and between facilities to achieve care objectives, acting as a liaison among primary care providers, specialists, and patients.
  • Advocates for patient access to essential services.
  • Adheres to State and County regulations governing nurse case management programs.
  • Engages in outreach efforts to the target population as directed.
  • Recommends modifications to programs or services to address community service gaps.
  • Performs additional duties as assigned.

Minimum Qualifications:
  • Current RN licensure in the practicing state.
  • Preferred minimum of one year of experience in Case Management.
  • Adheres to all relevant professional standards of practice.
  • Completion of Amergis' Competency program as applicable.
  • Current CPR certification, if applicable.
  • TB questionnaire, PPD, or chest x-ray, if applicable.
  • Current Health certificate as per contract or state regulations.
  • Compliance with all federal, state, and local requirements.
  • Successful completion of new hire training relevant to the job site.
  • Understanding of patient confidentiality and HIPAA regulations.
  • Strong communication skills with the ability to effectively convey and receive information.
  • Proficiency in computer skills.
  • Must be at least 18 years of age.
