About the Role:

This position involves coordinating care services for individuals receiving Long-Term Services and Supports (LTSS). You will work closely with patients, families, caregivers, and healthcare professionals to develop personalized care plans that meet the individual's unique needs.

Your Responsibilities:

  • Assess patient needs and develop comprehensive care plans.
  • Identify and connect patients with community resources and services.
  • Coordinate with healthcare providers and social services agencies to ensure seamless care delivery.
  • Advocate for patients and their families to ensure their voices are heard and their needs are met.
  • Monitor patient progress and make adjustments to care plans as needed.

Requirements:

  • Bachelor's degree in Social Work, Nursing, Psychology, Special Education, Gerontology, or related field.
  • 2+ years of experience working in a healthcare setting, preferably in case management or care coordination.
  • 1+ year of experience working with individuals with long-term care needs.
  • Ability to travel within assigned regions to visit patients in their homes or other settings.

Preferred Qualifications:

  • Licensed Social Worker or clinical degree.
  • Background in managing populations with complex medical or behavioral needs.
  • Experience with electronic charting and care planning software.

Estimated Salary: $60,000 - $80,000 per year.
