Job Title: Home Health Occupational Therapist

LHC Group is seeking a skilled Occupational Therapist to join our team in Essex Fells, New Jersey.

Job Summary

The Occupational Therapist will be responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on this assessment, the Occupational Therapist will develop a treatment plan, perform interventions to improve patient well-being, and evaluate progress.

Responsibilities
  • Provide occupational therapy services within the scope of practice as defined by state laws, in accordance with the plan of care, using evidence-based techniques, and in coordination with other healthcare team members.
  • Evaluate patient functional status, body systems, and occupational therapy needs, consulting with physicians to develop the therapy plan of care.
  • Observe, record, and report patient response to treatment and changes in condition to the nurse supervisor and physician.
  • Instruct and inform healthcare team members, patients, and family/caregivers regarding the plan of care, functional limitations, and progress towards goals.
  • Visit patients within the service area of their respective agency and other nearby LHC agencies as needed.
  • Complete progress notes for each visit and submit them per LHC policy.
  • Develop OT plans of care within state-specific guidelines, reporting outcomes, goals, and anticipated frequency of care to the nurse supervisor within 24 hours of evaluation.
  • Participate in staff conferences and committees as requested by the agency.
  • May supervise home health aides per LHC policy if allowed by state regulation.
  • Provide supervision of the OTA as per LHC policy and state a written discharge summary and/or a written summary report on the patient's condition at least every 60 days.
  • Conduct patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.
  • Provide in-service education as requested by agency leadership.
  • Adhere to LHC Therapy Standards of Practice in provision of services to all patients.
  • Access education to maintain and enhance clinical knowledge and skills up to date with current standards of care for the profession and to maintain state-issued licensure.
  • Participate in a weekend rotation to provide services for their respective agency and other nearby LHC agencies as needed.
Education and Experience

License Requirements

  • Current Occupational Therapy licensure in the state of employment.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle, or public transportation.
