We're seeking a skilled Physical Therapist Assistant to join our interdisciplinary team at Grove Park Healthcare Center. As a Physical Therapist Assistant, you will play a vital role in the rehabilitation journey of our residents, working under the supervision of our Physical Therapist to implement treatment plans and provide exceptional care.
Key Responsibilities:- Assist in assessments and gather objective data for Section GG collection during screenings.
- Implement treatment plans, including exercises, modalities, and mobility training, as directed by the Physical Therapist.
- Monitor residents' progress and report any changes or concerns to the supervising Physical Therapist.
- Maintain accurate and timely documentation of resident progress and interventions provided.
- Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress.
- State licensure as a Physical Therapy Assistant.
- EMR and MS office applications.
- CPR/BLS certification.
- Knowledge of Section GG data collection and documentation requirements.
