Support Specialist Role
As a Support Specialist, you will play a vital role in assisting individuals with I/DD in achieving their personal aspirations. You will be responsible for providing comprehensive care and support, fostering independence, and creating a positive and enriching environment.
Duties and Responsibilities:- Provide personalized assistance with daily living activities, including grooming, bathing, hygiene, toileting, feeding, and communication.
- Accompany individuals to medical appointments, social events, leisure activities, and outings to support their engagement and well-being.
- Implement approved emergency procedures in the event of unforeseen circumstances, ensuring the safety and security of all individuals under your care.
- Collaborate with interdisciplinary teams to develop and implement individualized service plans (ISPs) that align with each person's unique needs and goals.
- Deliver training and support to individuals based on their ISP objectives, fostering their skills and independence.
- Maintain accurate and detailed records of medical visits, medication administration, personal care, and other relevant information in accordance with established standards.
- Prepare nutritious meals according to posted menus, ensuring dietary needs are met.
- Create a clean, safe, and welcoming environment by maintaining the home's cleanliness and reporting any maintenance issues promptly.
- Communicate effectively with supervisors, colleagues, and families to ensure seamless coordination of care and support.
