Job Summary

Responsibilities
  • Conduct home visits to assess patients' physical needs and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate patients' mobility, strength, and balance.
  • Implement evidence-based interventions, such as manual therapeutic exercises and gait training, to promote patients' independence and mobility.
  • Collaborate with nursing staff to develop and revise care plans, ensuring seamless communication and coordination of care.
  • Educate patients and their families on rehabilitative care and activities necessary to promote healthy living.
  • Accurately document patient progress and care management, utilizing a state-of-the-art touch pad tablet.
Requirements
  • A current Delaware Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
