**Practice Details:**
* 12-provider primary care practice with 8 Internal Medicine Physicians, 1 Internal Medicine Advanced Practice Provider, and 3 Pediatricians
* Specialists in pediatrics, dermatology, mental health, midwifery, obstetrics and gynecology, and podiatry
* On-site lab services, x-ray, mammography, and bone densitometry
* On-site M Health Fairview Pharmacy, M Health Fairview Rehab Services, and M Health Fairview Urgent Care
* Patient encounters average 18-22 patients per day
* Virtual care offered and encouraged via video, telephone, and through our electronic medical record
* 34 patient contact hours per week for full-time physicians
**Call Frequency:**
* 1:22 low-intensity phone consultation only with first-tier nurse triage
As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care. We are fully integrated with an electronic medical record (EPIC).

**Qualifications:**
* Graduate of an accredited ACGME/AOA in Internal Medicine/Pediatrics or residency training program
* Board certified/board eligible with the ABMS or AOA
* Licensed or ability to obtain Minnesota Board of Medical Practice licensure
* Ability to obtain and/or maintain DEA certification for the State of Minnesota
* Meet threshold criteria/qualifications for credentialing and privileges
