Job Summary

We are seeking a highly skilled and detail-oriented MDS Assistant to join our team at Gallatin County. As a key member of our care team, you will play a vital role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Assist with the completion of the Minimum Data Set form (MDS) for nursing home resident assessment.
  • Review individual resident charts for nursing, therapy, physician, and interdisciplinary team (IDT) documentation and assessments.
  • Conduct resident interviews and resident testing (e.g., vision, hearing, etc.).
  • Conduct staff interviews to collect and compile required information for use in assessments.
  • Assist with completion of appropriate MDS assessment report for each resident, incorporating all information collected during interviews, chart reviews, and other data collection activities.
  • Complete audits, including but not limited to: Pressure Injury Audit, AIMS and Behavior Quarterly Assessments.
  • Assist with updating resident care area narrative assessments and care plans to ensure compliance with established timelines and guidelines.
  • Review charts and MDS assessments, conduct staff and resident interviews, and develop preliminary care plans.
  • Forward preliminary care plans to other care team members and nursing staff for input and approval.
  • Enter admission orders for new residents/patients.
  • Assess admission orders for possible contraindications, omissions, irregularities, and report to staff for follow-up.
Requirements
  • Must possess a valid driver's license issued by the state of Montana and be insurable under the county's liability policies.
  • As a condition of hire, pre-employment drug testing may be done.
  • Must be willing to work in a smoke-free environment.
  • As a condition of hire, the final candidate will be required to successfully pass a criminal history check and background investigation.
