Job Summary

The Pediatric Behavioral Health Navigator is a critical role in our organization, responsible for connecting pediatric patients and their families with necessary resources and support. This position requires a strong understanding of community health resources and the ability to navigate complex systems to ensure seamless care coordination.

Key Responsibilities
  • Provide intensive initial contact for each patient, with a minimum of one post-intervention follow-up call, and more for those needing additional support.
  • Develop and maintain a comprehensive listing of vetted area behavioral health providers.
  • Establish relationships with local mental health centers, pediatricians, and clinical outcomes management staff.
  • Conduct biopsychosocial intake assessments and coordinate subsequent referrals.
  • Provide flexible scheduling for consultation with referring physicians, other healthcare providers, and families.
  • Serve as a liaison between patients, families, and care providers.
  • Responsible for outreach efforts to establish and maintain positive working relationships with physicians, social services staff, and the broader health community.
  • Encourage patients to establish stable relationships with primary care providers and mental health providers.
  • Assist in increasing access to high-quality, culturally competent care through stable insurance coverage, transportation, and other needed social supports.
  • Facilitate communication and follow-up with healthcare providers.
  • Participate in the ongoing development and review of the program.
  • Participate in the collection and analysis of outcomes data.
  • Responsible to comply with all pertinent regulations under HIPAA.
Requirements
  • Master's degree in Social Work or related health field.
  • Minimum of 2-5 years of experience working within community behavioral health systems.
  • Level II background check required.
  • Obtain certification in the appropriate specialty within one year of being hired.
  • Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
  • Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
