Key Responsibilities
  • Develop and implement individualized patient care plans to meet the unique needs of medically complex patients.
  • Provide direct and indirect patient care, including assessment, planning, implementation, and evaluation of patient outcomes.
  • Supervise and coordinate the care of patients, ensuring seamless transitions and continuity of care.
  • Document patient progress and outcomes, using identified nursing diagnoses and interventions.
  • Maintain confidentiality and adhere to HIPAA guidelines at all times.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care and achieve optimal patient outcomes.
Requirements
  • Current RN license for the state in which the nurse practices.
  • One year of experience in a long-term acute care facility, with a focus on complex patient care.
  • Experience in acute rehabilitation, including vents, wound care, cardiac, and stable post-trauma patients.
  • Compliance with all relevant professional standards of practice.
  • Participation in Amergis' Competency program, as applicable.
  • Current CPR certification, if applicable.
  • TB questionnaire, PPD, or chest x-ray, if applicable.
  • Current Health certificate, as per contract or state regulation.
  • Meet all federal, state, and local requirements.
  • Successful completion of new hire training, as applicable to the job site.
