Job Title: PCHH RN Case Manager

At Katy Trail Community Health, we are seeking a dedicated and compassionate RN Case Manager to join our team in Sedalia. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.

Job Summary:

We are looking for a skilled RN Case Manager to work with our patients in Sedalia and Marshall. In this role, you will be responsible for creating individualized patient care plans, coordinating services, and collaborating with our multidisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Conduct patient assessments and develop comprehensive care plans
  • Coordinate services and collaborate with our healthcare team
  • Document patient information and follow up on care plans
  • Work closely with patients and their families to ensure continuity of care
Requirements:
  • Valid RN licensure in Missouri
  • At least three years of experience in a clinic setting
  • Experience in a community health center setting is preferred
  • Proficiency in utilizing various software and tools
