  • Collaboration with Healthcare Professionals: Work closely with physicians, registered nurses, and physical therapists to provide comprehensive care to patients.
  • Communication: Communicate with patients, families, and other healthcare professionals to ensure that everyone is aware of the patient's care plan, progress, and any changes in condition.
  • Supervision: May be responsible for supervising nursing assistants or other support staff to ensure that all patients receive proper care and that the facility runs smoothly.
  • Education: Provide education to patients and their families regarding medications, treatments, and care instructions.
  • Adherence to Policies and Procedures: Adhere to all facility policies and procedures, as well as state and federal regulations, to ensure the safety and well-being of patients.
Qualifications
  • Possession of a current license to practice practical nursing in the State of NY
  • A current certification in accordance with the American Heart Association's Basic Life Support (BLS) for Healthcare Providers
  • One year of experience in Long Term Care or SNF experience
