We are seeking a highly skilled and experienced Senior Registered Nurse to join our team at Central Counties Center for Mental. This is a full-time position that requires a strong background in nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Key Responsibilities- Provide advanced nursing care to individuals with intellectual and developmental disabilities, including medication administration, wound care, and vital sign monitoring.
- Develop and implement individualized care plans, in collaboration with the interdisciplinary team, to meet the unique needs of each consumer.
- Conduct comprehensive nursing assessments, including physical and psychosocial evaluations, to identify areas of need and develop targeted interventions.
- Collaborate with the healthcare team to ensure seamless transitions of care, including hospitalizations, emergency department visits, and community-based services.
- Provide education and training to consumers, families, and staff on various topics, including medication management, wound care, and health promotion.
- Participate in quality improvement initiatives, including data collection, analysis, and implementation of evidence-based practices.
- Develop and maintain relationships with community-based providers, including hospitals, clinics, and home health agencies, to ensure comprehensive care coordination.
- Current licensure as a Registered Nurse in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact.
- Minimum of 2 years of experience in nursing, with a focus on intellectual and developmental disabilities.
- Strong communication and interpersonal skills, with the ability to work effectively with consumers, families, and staff.
- Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
- Proficiency in electronic health records, including medication administration records and care plans.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Master's degree in nursing or a related field.
- Experience with trauma-informed care and cultural sensitivity.
- Knowledge of community-based services, including home and community-based services and Medicaid waiver programs.
- Ability to speak multiple languages, including Spanish.
