Our team provides comprehensive services, including mental health counseling, social work, certified nurse practitioner services, case management, and psychiatry.

We are seeking a Community-Based Therapist Professional to join our team. As a Disability Support Services Provider, you will work closely with clients and their families to provide personalized care and support.

Responsibilities:
  • Develop and implement treatment plans tailored to each client's goals and objectives.
  • Provide patient visits as scheduled, ensuring regular communication with clients and their families.
  • Maintain accurate and up-to-date records of client progress and treatment plans.
  • Collaborate with colleagues to provide holistic care and support to our clients.

Qualifications:
  • Bachelor's degree in Social Work or a related field, or Master's degree with relevant experience.
  • Social Work/Professional Counseling License or equivalent: LSW, LISW, LISW-S, LPC, LPCC, LPCC-S.
  • Experience working with individuals with developmental disabilities preferred.
  • Valid driver's license and reliable transportation required.
