Patient Navigator

Sun River Health is a network of Federally Qualified Health Centers providing comprehensive primary, dental, pediatric, OB-GYN, and behavioral health care to over 245,000 patients annually. We are seeking a part-time Patient Navigator to support our Beacon, NY site.

• Identify individuals and/or members of their families within the target population who are in need of preventive and primary health care services and link them to the services they need.
• Consult with medical staff to link patients to services through the provision of ancillaries such as translation, transportation assistance, and application assistance.
• Facilitate patient appointments within different departments of SRH and outside agencies, following SRH procedures for referring patients to different departments of SRH and outside agencies.
• Assist patients in overcoming obstacles that would prevent them from receiving needed referrals.
• Responsible for facilitating patients to appropriate referred agencies, with specific focus on referrals to specialty care and public health insurance programs, as needed and appropriate.
• Answer patient questions to clarify provider instructions, procedures, and referral needs.
• Responsible for transmitting all necessary clinical information available to specialist for referral, obtaining consultant reports, and obtaining consent for release of information.

• High school diploma/GED
• 2-4 years of work-related experience
• Bilingual Spanish is preferred
