Maxim Healthcare Services is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in a home healthcare setting.

The successful candidate will be responsible for applying the nursing process to assess, plan, implement, and evaluate patient care, under the supervision of a Registered Nurse (RN).

Key responsibilities include:

  • Utilizing the nursing process to assess patient needs and develop individualized care plans.
  • Performing focused interviews to identify specific patient needs and assessing signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
  • Collecting, analyzing, and interpreting data and information from healthcare members and documenting actual and/or potential nursing diagnoses.
  • Collaborating with the healthcare team to ensure seamless patient care.

Requirements:

  • Current Licensed Practical Nurse (LPN) License in the state in which the LPN practices.
  • Current Health Certificate (per facility).
  • Current PPD or Chest X-Ray.
  • Current BLS card.
  • One year prior LPN experience preferred.
