Responsibilities:
- Coordinate patient care, ensuring ongoing assessment of each patient's and family's needs while implementing the interdisciplinary care plan.
- Deliver services in accordance with physician orders and the established plan of care (POC).
- Ensure compliance with federal and state regulations in care delivery.

Qualifications:
- Graduate of an accredited clinical education program.
- Licensed to practice in the state of employment.
- Minimum of one year of experience as a licensed professional.
- Basic technology skills, including mobile device operation.

Preferred Experience:
- Medicare home health or hospice experience.

Requirements:
- Valid state driver's license.
- Automobile liability insurance as mandated by law.
- Reliable transportation in good working condition.
- Ability to drive safely in various weather conditions.
- CPR certification for healthcare providers.
