Position Overview
  • Illuminate Patient-Centered Care: Utilize patient-centered care to help build rapport and improve the health of acute care patients.
  • Champion Continuity of Care: Complete hospitalization follow-ups using the Azara system, manage diabetic care, and ensure follow-ups with ophthalmology and vision.
  • Navigate Complex Communications: Handle complicated messages from providers regarding patients' health, conduct nurse visits, and provide education.
  • Collaborate with Specialty Providers: Work with specialty providers, complete intakes and scribing, obtain medical records, and order diagnostic imaging.
  • Streamline Patient Support: Order supplies for patients via Parachute Health, write letters for providers, and complete phone triage for patients with emergent concerns.
Requirements
  • 3+ years of ER experience
