The ideal candidate will possess excellent communication skills, a strong work ethic, and a commitment to providing exceptional patient care.
Responsibilities:- Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions.
- Collaborate with the interdisciplinary team to develop and implement individualized patient care plans.
- Conduct thorough assessments of patients, identifying their physical, emotional, and social needs.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Current, valid LVN licensure in the State of California.
- Current BLS certification.
- At least six months of prior LVN experience in a general acute care hospital setting or acquired equivalent competency.
- Ability to read, analyze, and interpret common scientific and technical information, and to be easily understood through verbal communication in the English language.
