Job Description

The Medical Discharge Planner will conduct rounds with physicians to meet with patients and discuss placement orders. They will assist patients with developing a specific plan for medication management, include them in discharge planning, and help them identify a support network and instructions for future care and follow-up.

The ideal candidate will have a bachelor's degree in a human service field and knowledge in the development of discharge plans and coordination with community service providers. A valid CPR card is also required.
